Medical Monitoring <br />Clear Heart Drilling's personnel participating in field activities are included in a medical <br />monitoring program in conformance with 29 CFR 1910.120. The program includes a <br />baseline physical examination, pulmonary function test and blood and urine tests. <br />Annual follow-up and exposure incident examinations are included. Details of the <br />medical program are included. <br />Site Facilities <br />Restrooms are usually available at the site. Personnel conducting the tasks associated I <br />with this project shall not exit the site or enter the building unless proper decontamination <br />has occurred. <br />Safety Briefines <br />At the beginning of each workday the Site Safety Officer will brief all field staff as to �he <br />work conditions and hazards associated with that area. The nearest telephone will be <br />identified and the route to the nearest medical facility will be reviewed. The Site Safe�y <br />Officer will verify that all workers have reviewed the "Site Health and Safety Plan <br />Summary." The "Daily Safety Meeting Report" will be completed to serve as a record of <br />the "Site Safety Meeting" and the "Drill Rig Safety Inspection." See appendix A and i <br />appendix B. <br />4.0 EMERGENCY RESPONSE AND CONTINGENCY PLAN <br />The nature of work at the job site makes emergencies a continual possibility. Clear Heart <br />Drilling's Site Safety Officer will be familiar with emergency procedures and evacuation <br />routes. A first aid kit and emergency wash water will be readily available. <br />No significant chemical emissions resulting in acute exposure are anticipated at the site.. <br />If an injury occurs due to an accident, the Site Safety Officer will be immediately notif ed <br />so appropriate first aid can begin and medical attention arranged, if necessary. The Si <br />t$ <br />Safety Officer will investigate the nature and cause of the accident so that work <br />procedures can be modified to minimize the likelihood of the incident's recurrence. The <br />Site Safety Officer will notify the corporate Safety Officer and assist in the preparation of <br />required reports of injury or illness as required. <br />
