CAD 990 794 133 Modification 0 (g) Comeliaice Monitorin Pro ram If the Owner/Operator is required to establish a compliance monitoring program under Permit Condi- tion III.1(a) , he or she must, at a minimum, dis- charge the following responsibilities: (1) Notify the Director in writing within seven calendar days. (2) Immediately sample the ground water in all monitoring wells where analyses indicated a change from background concentrations and determine the concentration of all con- stituents identified in Appendix IX of 40 CFR 261 using sampling procedures identified in Permit Condition III.3. [40 CFR 264.98 (h) (2) ] (3) Within 90 days, submit to the Director an ap- plication for a permit modification to estab- lish a compliance monitoring program. [40 CFR 264.98(h) (4) ] The application must in- clude the following information: (i) An identification of the concentration of each Appendix IX (40 CFR 264) con- stituent found in the ground water at each monitoring well sampled in accor- dance with VII.A.l.a(1) (b) . [40 CFR 264.98 (h) (4) (1) ] (ii) Any proposed changes to the ground water monitoring system at the facility neces- sary- to meet the requirements of com- pliance monitoring as described- in 4()-- CFR 264.99. [40 CFR 264 .98 (h) (4) (ii) ] Any proposed changes to the monitoring frequency, sampling and analysis proce- dures, or methods or statistical proce- dures used at the facility necessary to meet the requirements of compliance monitoring as described in 40 CFR 264.99. [40 CFR 264.98 (h) (1) (iii) ] (iv) For each hazardous constituent found at a compliance point, a proposed con- centration limit, or a notice of intent to seek an alternative concentration limit for a hazardous constituent.
