9.3 Level 3 Incident <br /> Level 3 incidents range from minor to moderate emergencies that may require the assistance of emergency services <br /> (e.g., fire, police, etc.). Responding CE employees are responsible for assessing the scene and immediately escalating <br /> the incident to the appropriate company personnel and emergency services when needed. During Level 3 incidents <br /> requiring emergency services,the responding CE employee(or ranking employee)shall assume the role of the Incident <br /> Commander(IC) until relieve by emergency services.All level 3 incidents require a complete incident investigation,and <br /> a written report submitted to the CE Corporate Safety Manager within 24 hours of occurrence. <br /> Level 3 Incidents include, but are not limited to: <br /> ❑ Vehicle collision (with injury) <br /> ❑ Employee injury(requiring hospitalization) <br /> ❑ Customer injury <br /> ❑ Subcontractor incident <br /> ❑ Hazmat spill (minor) <br /> ❑ Fire Department response <br /> ■ Level 3 Incident Response <br /> 1) Incident notification received by the CE Command Center <br /> 2) Dispatch a CE Service Technician <br /> 3) Contact 911 (as needed) <br /> 4) Contact the CE Operations Supervisor <br /> 5) Contact the CE Corporate Safety Manager <br /> 6) Contact the CE Regional Director/Regional VP <br /> 7) Contact the VP of Operations <br /> 9.4 Level 4 Incident <br /> Level 4 incidents range from severe to catastrophic emergencies in which responding personnel cannot prevent harm <br /> to other personnel or equipment by taking reasonable actions such as engaging an ESD or isolating a system. During a <br /> Level 4 incident, the responding CE employee (or ranking employee) shall assume the role of the IC until relieved by <br /> emergency services.Once the area is secure,the responding employee shall perform a complete incident investigation, <br /> and submit a written report to the CE Corporate Safety Manager within 24 hours of occurrence. <br /> Level 4 Incidents include, but are not limited to: <br /> ❑ Station fire <br /> ❑ LNG Tank failure <br /> ❑ CNG bottle failure <br /> ❑ Hazmat spill (major) <br /> ❑ Fatality <br /> ❑ LNG trailer roll-over <br /> ❑ CNG vehicle fire <br /> • Level 4 Incident Response <br /> 1) Incident notification received by the CE Command Center <br /> 2) Dispatch a CE Service Technician <br /> 3) Contact 911 <br /> 4) Contact the CE Operations Supervisor <br /> 5) Contact the CE Corporate Safety Manger <br /> 6) Contact the CE Regional Director/Regional VP <br /> 7) Contact the VP of Operations <br /> L/CNG Emergency Response Plan �, Clean Enerav- 8 <br />
