9. This Permit Modification authorizes operation of the <br /> unit and listed activities expressly cited in attachment B, which- <br /> is attached hereto and by this reference incorporated herein. <br /> Any modification or addition to the unit and listed activities <br /> expressly described in Attachment B requires the Department's <br /> approval of a Permit Modification. <br /> 10. In response to the Department's requirement to <br /> implement a Corrective Action Program, Permittee submitted to the <br /> Department a revised post-closure maintenance plan for waste <br /> Management Unit A. By issuance of this Permit Modification, such <br /> revised plan, entitled "Revised Post-Closure Maintenance Plan for <br /> Waste Management Unit A," and all exhibits and attachments <br /> thereto, prepared by Kleinfelder, Inc. and Erler & Kalinowski, <br /> Inc. , dated November 4, 1994 (hereafter, the Approved Post- <br /> Closure Plan) , are hereby approved. The Approved Post-Closure <br /> Plan and any subsequent revisions thereof (such revisions are <br /> subject to the approval of the Facility Permitting Branch Chief) <br /> are by this reference made part of, and are hereby deemed to be <br /> requirements of, this Permit Modification. The Approved <br /> Post-Closure Plan hereby replaces all previous approved <br /> post-closure plans for the facility. <br /> 111. The Approved Post-Closure Plan establishes a two-year <br /> proof period to commence upon the effective date of this Permit <br /> Modification, during which trends in groundwater contaminant <br /> occurrences will be assessed.
