Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: COGENUJ36 Site Location: IN FRONT OF 1049 S WILSON WAY <br />Activity Summary <br />Activity Date Recorded by <br />07/28/2021 Received an email from Celia Barajas (CPM3@pge.com) with a copy of the MURO <br />analytical lab results, shipping papers, and a photo of the secured spill site taken <br />on evening of 7/27/2022. Photo shows the oil spill area was covered with visqueen <br />and secured with tape and cones. Celia wrote: "PSC will be onsite today to finish <br />additional cleaning of impacted soil and then our contractor will be conducting <br />confirmation sampling in next few days." <br />Reviewed Analytical Lab PCB Summary for three one-ounce samples of oil taken <br />from each of three impacted transformers on 07/27/2021. <br />Castle Analytical Laboratory Results: All tests used: EPA SW846 Method 8082 I <br />Same Day Oil Matrix I Practical Quantitation Limit (PQL) = 1.0 PPM <br />Lab ID: 2107087-01 A = 650 PCB Result PPM <br />Lab ID: 2107087-02 B = 7.6 PCB Result PPM <br />Lab ID: 2107087-03 C = 1260 PCB Result PPM <br />Updated Elena Manzo on complaint status. <br />Duplicate complaint # COUFBGUIO for the same incident was generated on <br />7/27/2021 because there was two different spills and two different responsible <br />parties from the same traffic incident. The responsible party for Complaint # <br />COGENUJ36 decided to take responsibility for the cleanup associated with <br />duplicate complaint # COUFBGUIO. Duplicate complaint will be abated. <br />07/29/2021 Completed Prop 65, Emergency Response Record, and site map. MURO <br />Received Cal OES Spill Update with updated PCB concentrations for the three <br />transformers. <br />Requested PCB cleanup plan from Celia Barajas (PG&E). Celia replied and asked <br />that I direct those questions to Zantha Ricks (PG&E Senior Environmental <br />Scientist, HAZMAT Programs) 925-597-7321. <br />Called Zantha, she reported she will be on site Monday to access extent of the <br />spill. She has notified US EPA Region 9 and has been in contact with them and <br />will be writing a report; asked if we would like a copy. They will be contracting <br />Arcadis for the confirmation sampling. Timeline to scheduling of confirmation <br />sampling is about one week. She reports they have no written procedure for the <br />confirmation sampling, but they intend to use the five-foot grid under Toxic <br />Substances Control Act (TSCA) which is in accordance with Hazardous Waste <br />Test Method SW-846. <br />07/29/2021 Updated Lead Senior Elena Manzo (EHD) on status of complaint. MURO <br />Revised Prop 65 per Elena's request. <br />08/03/2021 Called Zantha Ricks (PG&E) 925-597-7321 for update. PG&E will begin phase 1 <br />of the confirmation sampling on Wednesday, 8/11/2021. Zantha said the <br />confirmation sampling would be done in two phases: Phase One would be <br />confirmation sampling of the outer area such as sidewalk, street, and driveway. <br />And Phase Two would be confirmation sampling of the gutter and concrete. They <br />do not have written procedures; however, they conform with 5W846 test methods <br />and US EPA has given her a verbal approval.
