Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: COGENUJ36 Site Location: IN FRONT OF 1049 S WILSON WAY <br />Activity Summary <br />Activity Date Recorded by <br />08/05/2021 Called Zantha Ricks to inquire about the link she was going to email. Zantha was MURO <br />at the spill site on 8-2-2021 and confirmed the site is still covered and secured. <br />Zantha emailed link to regulations for higher than 500 ppm PCB spills and <br />scheduling for confirmation sampling on Wed., Aug 11. <br />https://www.ecfr.gov/cgi-bin/text-idx? <br />SID=552faf47f8c4e37855iff161a3e92f6e&mc=true&node=sp40.34.761.g&rgn=div <br />6 <br />PG&E will begin phase 1 of the confirmation sampling on Wed., Aug 11. Per our <br />telephone conversation on Aug. 3rd, Zantha said the confirmation sampling would <br />be done in two phases: Phase One would be confirmation sampling of the outer <br />area. And Phase Two would be confirmation sampling of the gutter and concrete. <br />She said they do not have written procedures; however, they conform with SW846 <br />test methods and US EPA gave her a verbal approval for it. <br />08/09/2021 Received US EPA contact information from Zantha Ricks (PG&E) on 8-6-2021. Zantha indicated that start time is 8 am and PPE level for sampling crew <br />is D with Tyvek booties and gloves. Zantha indicated that no inhalation hazard <br />related to PCB is anticipated. <br />08/11/2021 8:00 AM Onsite with Lead Senior REFIS Elena Manzo (EHD) and EHS Paul Nso <br />(EHD) in Level D PPE and Tyvek booties per recommendation from Zantha Ricks <br />(PG&E). Met with Jocelyn Wong (Arcadis) Mobile #: 415-710-0074 for <br />confirmation sampling event. I sent photos of PCB oil and Diesel spill sites before <br />cleanup to Jocelyn via text to her mobile phone. Jocelyn went over the plan for <br />confirmation sampling over the PCB Oil area of investigation. Arcadis drilled small, <br />excavated areas spaced five feet apart on sidewalk, street, and driveway and <br />collected bottom samples and placed in 4-ounce sample jars. Two samples were <br />collected from the storm drain: one destructive sample from the concrete inside <br />the surface grate and one wipe sample from inside the drain's lateral PVC pipe. <br />After the samples were collected, the excavated drilling was filled with a cement <br />mixture. Samples were labeled and placed on ice for transport to Excelchem, <br />Laboratories, Inc with a 48-hour rush request. <br />Took photos of samples, site plan, and chain of custody. <br />Left site at conclusion of sampling event. <br />08/11/2021 On site with lead senior Elena Manzo and REHS Claudia Muro for soil sampling. <br />Meet with sampler Jocelyn Wong tel 415 700 0074 of Arcadis and discuss <br />sampling plan. Observed beginning of sampling for PCB.
