VES R S <br />Contingency Plan <br />Section 12 — <br />Reporting / Critique <br />Effective Date <br />9/20/2021 <br />Supersefl,• <br />Ne \iv <br />Page <br />12-2 <br />CONTINGENCY PLAN <br />REPORTING REQUIREMENTS <br />Hazardous Waste - (40 CFR PART 264.56(i)) <br />The owner/operator must note in the operating record, the time, date and details of <br />any incident that requires implementation of the Contingency Plan. Within 15 days <br />after the incident, he/she must submit a written report on the incident to the <br />Regional Administrator and the Director of the appropriate State agency regulating <br />hazardous wastes. The report must include: <br />Name, address, and telephone number of the owner or operator. <br />Name, address, and telephone number of the facility. <br />Date, time, and type of incident (e.g., fire, explosion). <br />Name and quantity of materials involved. <br />The extent of injuries, if any. <br />An assessment of actual or potential hazards to human health and the <br />environment, where this is applicable. <br />Estimated quantity and disposition of recovered material that resulted from the <br />incident. <br />II. CRITIQUE <br />Critiques of the plan, emergency and follow-up response will be held as <br />determined necessary, on a case-by-case basis, by the Regional Operations <br />Manager or Regulatory Manager.
