Immediately contact a properly <br /> licensed,trained, and certified contractor to address the water intrusion into the 87, diesel STP and 91 fill sumps, and <br /> the 1/2, 3/4, 5/6, 7/8, and 11/12 UDCs under permit and inspection of the EHD. <br /> This is a repeat violation (Class 2) Violation. <br /> 53 CA Health and Safety Code 25292.2; CCR 2711(a)(11)2010007-Submitted/maintained current Cert of Financial <br /> Responsibility/mech of financial assurance(TCR 11). <br /> OBSERVATION: Current financial responsibility documents were not found on site. <br /> REGULATION GUIDANCE:A copy of current financial responsibility documents are required to be maintained on site. <br /> CORRECTIVE ACTION: Immediately obtain a copy of the facility's financial responsibility documents and maintain a <br /> copy on site. <br /> This is a repeat violation (Class 2) Violation. <br /> 55 CA Code of Regulations 2716(f)2010004-Copies of DO inspections for last 12 months maintained, 36 months for <br /> those after 10/1/18 (TCR 12). <br /> OBSERVATION:A designated operator inspection report was not found on site. <br /> REGULATION GUIDANCE: Designated operator inspection reports prior to October 1, 2018 shall be retained on site <br /> for twelve months. Designated operator inspection reports on or after October 1,2018 shall be retained on site for <br /> thirty-six months. <br /> CORRECTIVE ACTION: This violation was corrected on site by the DO after adding the missing report to the facilities <br /> records. <br /> This is a repeat violation (Class 2) Violation. <br /> 56 CA Code of Regulations 2716(a-e)2030013-DO inspections completed at least every 30 days and meet all <br /> requirements(TCR 12). <br /> OBSERVATION:The designated operator(DO)failed to document all the alarms and check that they were responded <br /> to appropriately. Missing alarms: L8-87 Fill 01/17/24,12/29/23; L10-91/DSL annular 08/31/23; Q—DSL LLD GLF <br /> 01/16/24, 08/30/23, 08/27/23, 08/21/23, 08/11/23 <br /> DO failed to check and note the correct test dates (10/18/21)for the overfill protection equipment <br /> DO failed to check and note all required items, including: <br /> -NA was marked in section 8, regarding whether alarms were responded to ,when there were alarms in the DO report <br /> -NA was marked for overfill testing having been completed in the last 36 months <br /> REGULATION GUIDANCE: During the inspection, the DO shall review the alarm history since the previous inspection, <br /> check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the inspection report.
