A dOAQU1N Environmental Health Department <br /> COUNTY <br /> CP22 - UST FULL Inspection Report <br /> Facility Name: Facility Address: Date: <br /> NASHIR EL DORADO INC 713 N EL DORADO ST, STOCKTON May 30, 2024 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> all required items on the report <br /> CORRECTIVE ACTION: Ensure that DOS performing inspections at this facility are including all of the required <br /> information on the reports <br /> OBSERVATION:The following were not attached to the DO inspection reports: <br /> Alarm printouts,work orders <br /> REGULATION GUIDANCE:The DO inspection report shall include: documentation demonstrating action taken in <br /> response to each compliance issue identified during the previous inspection-a copy of the alarm history since the <br /> previous inspection—a copy of documentation demonstrating action taken in response to each alarm since the previous <br /> inspection—a list of each alarm since the previous inspection-a list of each alarm since the previous inspection for <br /> which there is no documentation of the alarm condition and action taken in response <br /> CORRECTIVE ACTION: Ensure all required documents are attached to future DO inspection reports. <br /> This is a repeat violation (Class 2) Violation. <br /> 58 CA Code of Regulations 2715(c)(3), 2715(c)(4)2010015- Copies of Facility Employee Training Certificates maintained. <br /> OBSERVATION: The Facility Employee Training Certificate listed the incorrect facility name. <br /> REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if <br /> after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate <br /> shall contain accurate information for all employees trained. <br /> CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and <br /> submit a copy of the updated form to the EHD. <br /> This is (Minor)Violation. <br /> 59 CA Code of Regulations 2712(b)(1-2)2030001 - Records of release detection monitoring and maintenance properly <br /> maintained (TCR 9d). <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. Including: <br /> -Work orders for alarms 8/6/23, 8/3/23, 8/1/23, 7/26/23, 7/25/23, 7/24/23 87 annular high liquid alarm. Work orders for <br /> alarms 8/26/23, 8/10/23 91/D annular high liquid alarms. <br /> -Test results for secondary containment test conducted on 05/06/2022 (COS) <br /> -Test results of monitoring system certification conducted on 05/30/2023(COS) <br /> REGULATION GUIDANCE:These records shall be maintained on site for at least three years.
