The Facility Employee Training Certificate <br /> shall contain accurate information for all employees trained. <br /> CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and <br /> submit a copy of the updated form to the EHD. <br /> This is (Minor)Violation. <br /> 59 CA Code of Regulations 2712(b)(1-2)2030001 - Records of release detection monitoring and maintenance properly <br /> maintained (TCR 9d). <br /> OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. Including: <br /> -Work orders for alarms 8/6/23, 8/3/23, 8/1/23, 7/26/23, 7/25/23, 7/24/23 87 annular high liquid alarm. Work orders for <br /> alarms 8/26/23, 8/10/23 91/D annular high liquid alarms. <br /> -Test results for secondary containment test conducted on 05/06/2022 (COS) <br /> -Test results of monitoring system certification conducted on 05/30/2023(COS) <br /> REGULATION GUIDANCE:These records shall be maintained on site for at least three years.
