SEMONFIVE Health and Safety Requirements <br /> activities.All new employees must sign the Safety Compliance Agreement Form before beginning field <br /> work. <br /> In addition to the site-specific health and safety briefing,daily health and safety briefings will be <br /> conducted as necessary to address changes in procedures or other health and safety concerns.These <br /> periodic briefings will be documented using the On-Site Health and Safety Tailgate Meeting Record Form <br /> in Appendix B. <br /> 5.3 HEALTH AND SAFETY DOCUMENTATION <br /> The URS SHSO will document implementation of the HASP. The URS SHSO will set up a health and <br /> safety file containing health and safety related records and reports.This file will contain the following: <br /> • Signed copies of the Safety Compliance Agreement; <br /> • Calibration records for health and safety monitoring equipment; <br /> • Health and safety incident reports; <br /> • Records of safety violations and remedial actions taken; <br /> • On-Site Health and Safety Tailgate Meeting Record Form; <br /> Other information will be documented in a logbook including: <br /> • Weather conditions; <br /> • Site employees; <br /> • PPE level worn; <br /> • Monitoring instrumentation readings (average,peak,and background); and <br /> • Safety violations. <br /> Additionally, copies of the approved HASP(containing emergency phone numbers and a route to the <br /> hospital)will be on site in a location accessible to all project personnel. <br /> 5.4 MONITORING EQUIPMENT <br /> The following air monitoring equipment will be used during site activities: <br /> • Full Gas Monitor for Lower Explosion Limit, Carbon monoxide,Oxygen,and Hydrogen Sulfide. <br /> The instrumentation shall be calibrated daily prior to use and at the end of the day.The results of daily <br /> instrument calibrations shall be logged in the field note book. <br /> r <br /> UM <br /> H:\WPROCESS\25532\WORLDENTERPRISE\WELL DESTRUCTIOMATTACHAAENT C WORLD ENTERPRISES HSP TG EDITS.DOC\18-NOV-11\\ 5-2 <br />
