This reporting requirement must be adhered to without <br />exception for all incidents involving threats and assaults. The Director of <br />Public Works, or their designee, will, upon receiving notification, determine <br />the extent to which they will or will not become involved in the matter. In <br />those cases where an employee has been the victim of a threat or assault <br />perpetrated by his or her supervisor, the employee will report the incident to <br />the manager of the supervisor concerned, who will immediately report the <br />incident to the Director of Public Works, or their designee, <br /> Immediately following an incident, all parties to the incident should be <br />interviewed and written summaries prepared. In addition, every effort <br />should be made to get witnesses, including the participants, to make a <br />written statement. In most instances of actual physical violence, if possible <br />and with employee’s permission, take photographs of the employee’s <br />injuries. <br /> Any employee found to be responsible for a threat or assault is to be <br />immediately placed in a non-duty status (the Director of Public Works or <br />designee must approve) pending further investigation of the matter. <br /> If the situation does not appear dangerous, take the following actions: <br />o Separate employees involved and isolate until interviewed; <br />interview all employees who have information concerning the <br />incident immediately (individually) while impressions are still fresh in <br />their minds; <br />o establish circumstances leading to the incident; <br />o obtain written statements from each witness; and <br />o do not allow witnesses to discuss what happened with each other <br />before preparing the statements. <br /> If the situation involves a non-employee, and management is unsuccessful in <br />diffusing the situation, ask the person to leave. If they refuse to leave <br />contact 9-911 <br />o Reminder: The employee is responsible for reporting the violation to <br />their immediate supervisor or management. Management is <br />responsible for completing the report form. If the employee fails to <br />write a statement, this action does not negate managements’ <br />responsibility to report and investigate. <br />o The Director of Public Works, or their designee, may convene <br />members of the San Joaquin Threat Assessment Team (STAT), as <br />necessary in the judgment of the Director, in order to use their <br />expertise to assess the seriousness of any situation reported by <br />members of the Leadership Team.
