Revocable Permit No. RP24-0284 <br /> WSP USA ENVIRONMENT & INFRASTRUCTURE INC. <br /> Page 2 <br /> 3. The issuance of this permit by the City does not create any agency on the part of <br /> the Permittee, and the permittee will hold the City of Stockton harmless for any <br /> action for damages arising out of the Permittee's activities performed under this <br /> permit. <br /> 4. In accordance with Section 1(h) of the "Standard Conditions Applicable to <br /> Revocable Permits," the permittee shall provide and keep in force during the term <br /> of the permit a policy of public liability insurance against liability for injuries to <br /> persons or property arising out of accidents attributable to the structure on City <br /> property, in amounts to be determined by the Risk Manager. The policy of <br /> insurance so provided shall contain a contractual liability endorsement covering <br /> the liability assumed by the Permittee by the terms of his permit and shall contain <br /> a provision that such policy may not be canceled except after thirty(30) days notice <br /> in writing to be given to the Risk Manager. The copies of these policies or <br /> certificates evidencing the same shall be filed with the City Risk Department after <br /> approval of the City Attorney. <br /> 5. Permittee, or his contractor, shall obtain an Encroachment Permit for the <br /> installation of the monitoring well. <br /> 6. Future monitoring well sampling, maintenance, and/or removal shall require an <br /> additional Encroachment permit. <br /> 7. The well head shall be covered with a meter box or valve box as approved by the <br /> City of Stockton. The top of the box shall be flush with the surrounding ground or <br /> concrete. <br /> 8. The provisions of the State of California, Department of Water Resources Bulletin <br /> 74-81 and 74-90 entitled "Water Well Standards: State of California" shall be <br /> applicable to this permit. <br /> 9. Construction equipment that emits loud noise shall use muffling device. <br /> Construction shall be confined between the hours of 8:30 a.m. and 5 p.m. <br /> 10. Disposal of water from the well site shall be in accordance with current federal <br /> regulations. <br /> 11 . The barricading of any portion of City streets shall be subject to the approval of <br /> the City Traffic Engineer. Traffic cones and signage shall be used to direct traffic <br /> and shall be installed in accordance with the latest California Manual on Uniform <br /> Traffic Control Devices. <br />
