City and State <br /> • "REWARD IF FOUND" <br /> Some of the information listed above, such as the list of activities and ending dates, should <br /> be entered after the entire logbook has been filled or after decision that the remaining blank <br /> pages in the logbook will not be filled. <br /> The spine of the logbook should contain an abbreviated version of the information on the <br /> cover: for example, "1, Col.Ave., Hastings, 5/2015 - 8/2015." <br /> 2.2.2 First Page of the Field Logbook <br /> Spaces are usually provided on the inside front cover (or the opening page in some <br /> logbooks), for the company name ("Toeroek Associates, Inc."), address, contact name, <br /> telephone number, and email address. If preprinted spaces for this information are not <br /> provided in the logbook, write the information on the first available page. <br /> 2.3 Entering Information in the Logbook <br /> Enter the following information at the beginning of each day: <br /> • Date. <br /> • Starting time. <br /> • Specific location. <br /> • General weather conditions and approximate temperature. <br /> • Names of personnel present at the site, including visitors. Note the affiliation(s) and <br /> designation(s) of all personnel. <br /> • Plan of the Day. <br /> • Equipment calibration and equipment models used. <br /> • Health and Safety topic of the day and Daily Tailgate meeting. <br /> The following information should be recorded at each new site visited and as appropriate <br /> throughout the day <br />
