TOEROEK SOP No.017,REv.2 <br /> ASSociATE5,INC RECORDING OF NOTES IN FIELD LOGBOOK <br /> PAGE 5 <br /> • A general title of the task undertaken (for example, well installation at MW-11, <br /> decontamination at borehole BH-11, groundwater sampling at MW-11). <br /> • Description of the activities performed such that the activities could be recreated. <br /> This must be completed concurrently with field activities and cannot be completed <br /> subsequently. For instance, all activities for low-flow groundwater sampling must be <br /> recorded in the logbook as they happen, including water levels, all water quality <br /> parameter data, pump settings (e.g., depth of intake, flow), decisions concerning <br /> stabilization,all sampling information (e.g.,sample time,sample analyses,containers, <br /> preservatives), and any other relevant information. RECORDING INFORMATION ON <br /> FIELD DATA COLLECTION SHEETS ONLY IS NOT ACCEPTABLE BECAUSE (1) THE <br /> LOGBOOK IS THE LEGAL RECORD;AND (2)LOOSE SHEETS CAN BECOME LOST OR <br /> DESTROYED. TOEROEK HAS A ZERO TOLERANCE POLICY. It is acceptable to <br /> complete field data collection sheets after completion of the activities from the <br /> information in the logbook, but it is not acceptable to complete the logbook from the <br /> field data collection sheets. <br /> • Changes in instructions or activities at the site. <br /> • Levels of personal protective clothing and equipment. <br /> • Approximate scale for all diagrams. If this can't be done, write "not to scale" on the <br /> diagram. <br /> • Indicate the north direction on all maps and cross-sections. Label features on each <br /> diagram. <br /> • Corrections, if necessary, necessarily including a single line through the entry being <br /> corrected. Initial and date any corrections made in the logbook. <br /> • After last entry on each page, initials of the person recording notes. No information is <br /> to be entered in the area following these initials. <br /> • At the end of the day, signature of the person recording notes and date at the bottom <br /> of the last page. Indicate the end of the work day by writing "Left site at (time)." A <br /> diagonal line must be drawn across any remaining blank space at the bottom of this <br /> last page. <br /> The following information should be recorded in the logbook after taking a photograph: <br /> • Time, date, location, direction, and, if appropriate, weather conditions. <br /> • Description of the subject photographed and the reason for taking the picture. <br /> • Sequential number of the photograph and the film roll number or disposable camera <br /> used (if applicable). <br /> • Name of the photographer. <br /> • Do not delete any photographs, even if it was taken accidentally. The photograph is <br /> part of the log, but it will not necessarily be used in a report. <br /> The following information should be entered into the logbook when collecting samples: <br /> • Location description and sample number. <br />
