TOEROEK SOP No.017,REv.2
ASSociATE5,INC RECORDING OF NOTES IN FIELD LOGBOOK
PAGE 5
• A general title of the task undertaken (for example, well installation at MW-11,
decontamination at borehole BH-11, groundwater sampling at MW-11).
• Description of the activities performed such that the activities could be recreated.
This must be completed concurrently with field activities and cannot be completed
subsequently. For instance, all activities for low-flow groundwater sampling must be
recorded in the logbook as they happen, including water levels, all water quality
parameter data, pump settings (e.g., depth of intake, flow), decisions concerning
stabilization,all sampling information (e.g.,sample time,sample analyses,containers,
preservatives), and any other relevant information. RECORDING INFORMATION ON
FIELD DATA COLLECTION SHEETS ONLY IS NOT ACCEPTABLE BECAUSE (1) THE
LOGBOOK IS THE LEGAL RECORD;AND (2)LOOSE SHEETS CAN BECOME LOST OR
DESTROYED. TOEROEK HAS A ZERO TOLERANCE POLICY. It is acceptable to
complete field data collection sheets after completion of the activities from the
information in the logbook, but it is not acceptable to complete the logbook from the
field data collection sheets.
• Changes in instructions or activities at the site.
• Levels of personal protective clothing and equipment.
• Approximate scale for all diagrams. If this can't be done, write "not to scale" on the
diagram.
• Indicate the north direction on all maps and cross-sections. Label features on each
diagram.
• Corrections, if necessary, necessarily including a single line through the entry being
corrected. Initial and date any corrections made in the logbook.
• After last entry on each page, initials of the person recording notes. No information is
to be entered in the area following these initials.
• At the end of the day, signature of the person recording notes and date at the bottom
of the last page. Indicate the end of the work day by writing "Left site at (time)." A
diagonal line must be drawn across any remaining blank space at the bottom of this
last page.
The following information should be recorded in the logbook after taking a photograph:
• Time, date, location, direction, and, if appropriate, weather conditions.
• Description of the subject photographed and the reason for taking the picture.
• Sequential number of the photograph and the film roll number or disposable camera
used (if applicable).
• Name of the photographer.
• Do not delete any photographs, even if it was taken accidentally. The photograph is
part of the log, but it will not necessarily be used in a report.
The following information should be entered into the logbook when collecting samples:
• Location description and sample number.
