provide at no cost to State any laboratory reports on samples taken from or at the <br /> Premises, and written reports and supporting documentation provided by Permittee to the <br /> CVRWQCB or other regulatory agencies relating to Permittee's implementation of the <br /> Work Plans or access to the Premises. Said results shall be sent in a timely fashion via <br /> email to: <br /> tamer.ahmedCa.cdcr.ca.aov <br /> richard.tavlor(cDcdcr.ca.aov <br /> richard.delany(a)cdcr.ca.00v <br /> derrick.bowler(a-)cdcr.ca.gov <br /> tong.psihopaidas(cadas.ca.aov <br /> To the extent that email servers are unable to deliver data which exceeds delivery <br /> capabilities, Permittee shall provide data using a File Transfer Protocol (FTP) <br /> methodology or similar. <br /> Contamination Permittee shall take all measures necessary to ensure that their performance of the Work <br /> Plans does not contribute to or exacerbate the extent of any contamination on, around, or <br /> under the Premises, or result in any contamination of the Premises or the underground <br /> aquifer(s) beneath the Premises. In the event that Permittee's work does contaminate <br /> the Premises or aquifers, in addition to all other provisions of this Agreement, Permittee <br /> shall be liable to State for any damage that results from such contamination. Permittee <br /> agrees that their investigations at the Facilities will consist of those required to implement <br /> the Work Plans for Permittee's compliance with the Orders and for no other purposes <br /> whatsoever. Any attempt to complete investigations or activities outside of those <br /> necessary to implement the Work Plans without the written consent of the State will be <br /> deemed a violation of this Agreement. <br /> Well Removal At the completion of Permittee's monitoring activities, Permittee will, at its sole expense <br /> and in accordance with California Well Standards (DWR Bulletin Nos.74-81 and 74-90) <br /> properly decommission the wells at the Premises that are not needed for its monitoring <br /> program. <br /> GENERAL INSURANCE REQUIREMENTS: <br /> Insurance Permittee shall at all times during the performance of this Agreement, keep and maintain <br /> policies of Commercial General Liability Insurance, Automobile Liability Insurance, and <br /> Workers' Compensation Insurance with carriers rated A-VIII (or higher) by A.M. Best in <br /> amounts required herein. Likewise, Permittee's contractors shall at all times during <br /> performance under this Agreement keep and maintain Commercial General Liability <br /> Insurance, Automobile Liability Insurance, and Workers Compensation Insurance. <br /> Evidence of At least ten (10) days prior to entry upon the Premises, Permittee shall furnish to State a <br /> Insurance Certificate of Insurance from entities performing activities and services upon which the <br /> Right Of Entry Permit& Monitoring Well Agreement January 12, 2021 (L-2620) Page 8 <br />
