The Site Plan Appendix B illustrates all proposed improvements on the property. Section 10.9: The CGS owner will submit a copy of the service agreement with a licensed septage pumping firm. Section 10.10: Operational records, such as pump-out events will be maintained by the CGS owner and presented to EHD in the annual operating permit. Section 10.11: The CGS owner will obtain an annual operating permit from EHD. IX. OPERATIONS AND MAINTENANCE PLAN Operations will include those previously referenced, primarily gravity flow of influent from each individual source, force main pressure of effluent to individual Lift Stations, and ultimately to the Package Treatment Plant. Maintenance will generally consist of periodic cleaning based on direct observations of pump screens, filter media and purging filter bed laterals of biomat buildup. Control panels should be inspected and tested monthly to ensure proper audio and visual alarm operation. The following maintenance procedures are to be performed monthly/yearly, or a different time interval if determined by a Qualified Professional after the first inspection. 1. Observe a representative sample of septic tanks to check for solids buildup. 2. Check Lift Station duplex pumps and visual/audible alarm systems monthly. 3. Check grease interceptor tank and effluent filter for excessive grease content and potential pump- out intervals. 4. Check filter bed observation/monitoring tubes for standing effluent. 5. Check the Septic Tank Monitoring Systems on the west side of the Camp Gold Star facility for standing effluent within the 8" monitoring tube. 6. Check visual/audible alarm for the holding tank on a monthly basis and ensure no leakage of influent around the holding tank entry point or from pump out procedures. 7. Terminal ends of each lateral must be flushed out every two years or sooner if significant biomat buildup is observed during the first clean out. The effluent from this flushing must be captured in a container such as a bucket and the contents poured into the CGS holding tank. Signs should be placed in each Bathroom/Restroom signifying that CGS is on a septic system and nothing but human waste and biodegradable toilet paper(e.g., Scotts®Rapid-Dissolving Toilet Paper) are to be flushed down the toilets. The EHD requires an annual operating permit and potentially a biannual or annual inspection report on the Package Treatment Plant operation, which includes monitoring, sampling and reporting.
